If you are a new patient, please download and print our Patient Information Form and bring it with you to your appointment. Insurance
As a courtesy, we will file your insurance for you. If your insurance company is not listed below we would be happy to call and find out your benefits. We will need the name of the policyholder, date of birth, company name, and ID number. We are also able to bill most medical insurance companies for any medically-related visits to our office.
The Eyecare Center participates with most insurance companies as well as these common insurance companies:
